Job Summary

Dignified Nursing Home Health Care Agency, LLC is seeking an experienced Registered Nurse (RN) in the Houston area to complete home health skilled nursing visits (PRN/per visit). We are looking for a reliable, compassionate nurse who is confident working independently and completing accurate documentation in a timely manner.

Responsibilities

• Perform skilled nursing home visits and complete required assessments per Medicare guidelines
• Conduct comprehensive nursing assessments and documentation
• Monitor and report changes in patient condition to the supervising clinician/physician as appropriate
• Provide patient education and safety planning as needed
• Submit visit documentation on time and according to compliance standards

Requirements

• Active Texas RN license (in good standing)
• Minimum 2 years of current/recent RN experience in Home Health/Home Care (required)
• Reliable transportation, valid driver’s license, and active auto insurance
• Strong communication skills and professional bedside manner
• Ability to complete documentation accurately and promptly
• Current CPR/BLS certification
