Transform Advanced Illness Care.</strong></p> <p> </p> <p>We are seeking a dedicated a <strong>Remote Nurse Practitioner</strong> to join our Advanced Illness Management (AIM) team, covering our White Bear Lake, MN palliative care branch. In this role, via virtual/telephonic platforms, you will provide expert diagnosis, treatment, consultation, and follow-up for patients with complex needs, working collaboratively with the Professional Services Corporation (PSC) Physician, Primary Care Physicians, and interdisciplinary palliative care teams.</p> <p> </p> <ul> <li><strong>NP Licensure - Minnesota</strong></li> <li><strong>DEA Licensure, with experience writing prescriptions</strong></li> <li><strong>Hospice and/or palliative care experience preferred</strong></li> <li><strong>Schedule:  Monday-Friday, 9-6</strong></li> </ul> <p> </p> <p>Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity.  Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.</p> <p> </p> <p>Salary Range - $96,390 - $130,000</p> <h2>Essential Functions</h2> <ul> <li>Collaborate with PSC Physician, Primary Care Physician, and palliative care team members to deliver comprehensive advanced illness management and palliative care via telephonic and virtual platforms.</li> <li>Take full responsibility for patient care from initial assessment through discharge, addressing disease-specific changes, functional decline, symptom management, nutrition, cognition, and caregiver support.</li> <li>Develop individualized care plans in partnership with patients, families, and care teams.</li> <li>Facilitate access to treatments, services, and durable medical equipment as needed per care plan.</li> <li>Participate in evening and weekend call rotations providing clinically competent on-call services.</li> <li>Prescribe medications including controlled substances within licensure and state law.</li> <li>Order and interpret lab tests, imaging, and other diagnostics relevant to patient care.</li> <li>Maintain bi-weekly communication with the PSC Physician and update care plans based on ongoing assessment.</li> <li>Collaborate with social workers, RN care managers, and other supportive care providers to optimize patient outcomes.</li> <li>Ensure all clinical documentation meets standards to support medical practice and billing compliance.</li> <li>Monitor and report on palliative medicine quality measures and documentation requirements.</li> </ul> <h2>About You</h2> <p><strong>Specialized Knowledge and Skills:</strong></p> <ul> <li>Strong leadership and interpersonal communication skills.</li> <li>Independent decision-making ability and self-motivation.</li> <li>In-depth knowledge of palliative medicine principles, Medicare/Medicaid regulations, value-based care, and interdisciplinary processes.</li> <li>Meets continuing education requirements set by Gentiva and licensing boards.</li> <li>Proficiency with electronic medical record (EMR) software.</li> </ul> <p><strong>Education and Experience:</strong></p> <ul> <li>Master of Science in Nursing (MSN) or equivalent from an accredited institution.</li> <li>Minimum two years of Nurse Practitioner clinical experience.</li> <li>Specialty in Family or Adult Medicine preferred.</li> <li>Experience with value-based care models and Part B billing preferred.</li> <li>Ability to work autonomously in an advanced practice role.</li> </ul> <p><strong>Licenses, Certifications, and Agreements:</strong></p> <ul> <li>Current RN license or equivalent as required by state law.</li> <li>Active and Unrestricted Nurse Practitioner certification/license without board actions</li> <li>Collaborative agreement with supervising physician from the Professional Services Corporation.</li> <li>Part B Provider certification.</li> <li>Federal DEA license;
